Welcome to your Sprout Health journey! This article walks you through each step, from initial eligibility to refills, so you know what to expect and how to get the best results from your chosen treatment.
Step 1: Complete the Pre-Qualification Quiz
Begin by taking our short quiz to determine your eligibility for the medication you selected.
A successful quiz result pre-qualifies you, allowing you to move forward with purchasing your plan.
We take your privacy seriously: the information you share is HIPAA-compliant, encrypted, and only accessible to your care team.
Step 2: Purchase Your Medication Plan & Submit the Medical Questionnaire
After you purchase your medication plan, you’ll receive an email with a link to a detailed medical questionnaire.
Alternatively, you can access the questionnaire via the Medical Portal using the email you used for purchase. No password setup is required, just a one-time passcode (OTP) login.
This step gathers relevant health information so our licensed clinician can evaluate your suitability for therapy.
Step 3: Medical Questionnaire Review
Once submitted, your questionnaire is reviewed by an assigned licensed clinician.
If you do not qualify, your order will be cancelled and a refund issued.
If approved, the clinician issues a prescription which begins your treatment process.
Step 4: Order Preparation & Shipping
Upon prescription approval, your medication is sent to one of our partner pharmacies for fulfillment.
Orders are typically shipped within 3 – 7 business days.
Step 5: Track Your Order Status
Once shipped, you’ll receive tracking information via email.
You can also view your tracking details and order status in your Customer Portal (with the same credentials as your purchase email).
Step 6: Start Your Treatment
Your shipment will include everything you need to begin the medication. We recommend starting as directed.
Make sure you store and take the medication in accordance with the instructions. A digital guide will be emailed to you covering usage, storage, potential side-effects, and other important information.

Step 8: Contact Your Medical Provider
If you have clinical questions, about treatment, side-effects, dosage changes, please contact your licensed clinician via the Medical Portal.
Important: If you have used this medication previously and wish to continue at a specific dosage, let your clinician know during the consultation phase before the prescription is issued.
Step 9: Refilling Your Medication (Monthly or Quarterly)
Monthly Plan:
Your subscription is rebilled on the same date each month.
After each rebill, you’ll receive a new medical questionnaire which must be submitted for the clinician to assess your progress and any side-effects. Medication shipping depends on approval of this questionnaire.
Quarterly Plan:
Subscription is rebilled every three months.
For GLP-1 treatments: you’ll receive medication covering two months, and after the third dose in month two you’ll become eligible to submit the next questionnaire.
For NAD+ treatments: medication is month-by-month; a new questionnaire is submitted after the third dose each month. Sprout Health
